The doctors and staff of Mid-Ohio Valley Medical Group (MOVMG) would like to welcome you to our practice.  We
are pleased that you have entrusted us with your care and look forward to providing you with the highest quality
of health care for the entire family with compassion, integrity, and respect.

For your convenience, we have a patient portal that will allow you to request prescription refills, ask a
nurse/doctor a question, request an appointment or pay your bill on-line.


                                   IMPORTANT!
                           Please read before you begin to register online.

In order to create an account on the portal, please contact our office and give us an e-mail address to be put into
your account in Patient Registration.  Our system will send you an e-mail with a link in it to create an account
which will pull in much of the needed information.  From that link you will be able to create an account on the
Patient Portal.  Each patient must have their own account on the portal.  We can use the same e-mail address but
it must be in the individual patient’s account.   Access to the Patient Portal is through our website – www.
movmg.com.


  1. Provide and submit all required information.
  2. Click on “Health Forms”
  3. Provide and submit all required information (Preregistration Form, Health History Form, Patient                  
       Questionnaire, Consent for Disclosure of Health Care Information
  1. You will know that you are finished with each form when you reach the confirmation message.
  2. Arrive a few minutes early for your first appointment to make sure that all of your paperwork is in order.
  3. Remember to bring all of your insurance cards with you.

TIPS FOR LOGGING IN:  www.movmg.com

  1. Please set up a different account for each family member that is a patient at Mid-Ohio Valley Medical Group.
  2. Do not request an appointment or prescription renewal on anybody’s account except the patient’s account.
  3. Accounts can be set up for minor children.
  4. Please use Tabs appropriately:  Use the Request an Appointment Tab for appointments only.  Use the           
    Request Prescription Renewal Tab for prescriptions only.
  5. Contact a Practice Manager is for non-clinical questions, suggestions, and comments only.  If you have a
    medical question, please use the Contact a Nurse/Doctor Tab.
